To request your Certificate of Discharge, e-mail to the Clerk's Office with the following required information.
- First Name
- Last Name
- Case Number
- Last Four Digits of SSN. For your protection, do not send the full SSN.
Please note: the provided PDF document will contain a "Certificate of Notice" that shows all parties and creditors who have received written notice of the discharge (via USPS or email), a copy of the Discharge itself, and a general explanation of what a discharge means.
