E-Filing Proof of Claims (ePOC)

Proof of Claim forms for all chapters may be filed electronically through the Court's website. When submitted, the Proof of Claim form is automatically created and filed in the case. A CM/ECF login and password are not required.

The name and complete address of the filer must appear on the claim form. If an attorney is filing the claim on behalf of a creditor, the attorney should also enter his/her name and address. The name and title, if any, of the person authorized to file the claim is required on the claim form. The name should be preceded by "/s/" as, for example, "/s/ Jane Doe".

Penalty for filing a fraudulent claim: Fine of up to $500,000 or imprisonment for up to 5 years, or both. 18 U.S.C. Section 152 and 3571.

Withdrawing a Claim (eWOC)

A Notice of Withdrawal of Claim document must be prepared and uploaded by the filer. The document must be in PDF format and can be located here.

The name and complete address of the filer must appear on the Notice of Withdrawal of Claim. If an attorney is filing the notice on behalf of a creditor, the attorney should also enter his/her name and address. The name and title, if any, of the person authorized to file the claim is required on the claim form. The name should be preceded by "/s/" as, for example, "/s/ Jane Doe".
