Unclaimed Funds
Complete and follow instructions of the below forms for individuals, joint parties or entities. Please contact the court's financial administrator with questions at (515) 323-2844.
Signed documents need to be filed with the court either in person, by mail, express mail services or courier service at U.S. Bankruptcy Court, 300 U.S. Courthouse Annex, 110 East Court Avenue, Des Moines, IA 50309.
Copies of all documents need to be served and service certified as instructed on page 3 of the Motion for Payment of Unclaimed Funds.
Individual or Joint Parties:
- Motion for Payment of Unclaimed Funds
- Identification Form for Unclaimed Funds - Individual
- Limited Power of Attorney if you have granted another party the legal right to collect the funds on your behalf.
- Tax Identification Number Certification (NOTE: The Financial Information section of the form should not be completed.)
Entities:
- Motion for Payment of Unclaimed Funds
- Identification Form for Unclaimed Funds - Corporate/Business
- Limited Power of Attorney if entity has granted another party the legal right to collect the funds on it's behalf.
- Tax Identification Number Certification (NOTE: The Financial Information section of the form should not be completed.)
Local Forms
Local forms are drafted and approved by the Board of Judges for use in the Southern District of Iowa. If a form use has been designated as mandatory, failure to utilize the form may result in a delay in the processing of a case.
